Lead data not collected for Azure Marketplace

I have a solution published on Azure Marketplace with successfully enabled CRM integration. It is collecting customer's info when they are installing offer from the Azure Marketplace portal, but somehow stopped collecting customers data when they install it from Azure Portal. It worked originally several weeks ago and not working right now. I tested it with several different accounts and absolutely positive about it. In Azure Portal it's not even asking about personal details - just install solution to subscription.
